Software flaws happen because software is written very cheaply. We’re not building rocket ships, we’re building tools that let people manage their vacation photos. People want software that can competently allow them to do that for the cheapest price possible.

Software that does go into rocket ships (and airplanes, etc) is called High Integrity Software. It is produced in a completely different processes than business and consumer software; the programmers obsess about validity and painstaking review, test and validate every line. High Integrity Software has a very good safety record but is also very expensive to produce.
